Scholars and professional organizations in bioethics describe various approaches to “quality assessment” in clinical ethics. Although much of this work represents significant contributions to the literature, it is not clear that there is a robust and shared understanding of what constitutes “quality” in clinical ethics, what activities should be measured when tracking clinical ethics work, and what metrics should be used when measuring those activities. Further, even the most robust quality assessment efforts to date are idiosyncratic, in that they represent (...) evaluation of single activities or domains of clinical ethics activities, or a range of activities at a single hospital or healthcare system. Countering this trend, in this article we propose a framework for moving beyond our current ways of understanding clinical ethics quality, toward comprehensive quality assessment. We first describe a way to conceptualize quality assessment as a process of measuring disparate, isolated work activities; then, we describe quality assessment in terms of tracking interconnected work activities holistically, across different levels of assessment. We conclude by inviting future efforts in quality improvement to adopt a comprehensive approach to quality assessment into their improvement practices, and offer recommendations for how the field might move in this direction. Historians of science do not generally indulge in much mythologizing about the founders and prominent figures in our discipline's history. Nevertheless, the received history of the field elides a great deal of the work done on behalf of the Society and its publications because it was tedious, contentious, done in service to discarded aims, performed by underpaid or unacknowledged professional staff, or simply unglamorous. A brief tour through the Society's voluminous if poorly organized archives serves as a corrective.

The decision of the German regional court in Cologne on 26 June 2012 to prohibit the circumcision of minors is important insofar as it recognises the qualitative similarities between the practice and other prohibited invasive rites, such as female genital cutting. However, recognition of similarity poses serious questions with regard to liberal public policy, specifically with regard to the exceptionalist treatment demanded by certain circumcising groups. At a time of COVID-19 pandemic, universal basic income (UBI) has been presented as a potential public health ‘upstream intervention’. Research indicates a possible impact on health by reducing poverty, fostering health-promoting behaviour and ameliorating biopsychosocial pathways to health. This novel case for UBI as a public health measure is starting to receive attention from a range of political positions and organisations. However, discussion of the ethical underpinnings of UBI as a public health policy is sparse. This is depriving policymakers (...) of clear perspectives about the reasons for, restrictions to and potential for the policy’s design and implementation. In this article, we note prospective pathways to impact on health in order to assess fit with Rawlsian, capabilities and perfectionist approaches to public health policy. We suggest that Raz’ pluralist perfectionist approach may fit most comfortably with the prospective pathways to impact, which has implications for allocation of resources. (shrink)

The COVID-19 crisis has served not just to instil fear in the populace but to highlight the importance of fear as a motivating dynamic in politics. The gradual emergence of political-philosophical approaches calling for concern for ‘positive’ emotions may have made sense under non-pandemic conditions. Now, however, describing fear in the face of a deadly pandemic as ‘irrational’ or born of ‘ignorance’ seems ‘irrational’ and ‘ignorant’. In this article, we draw upon the work of John Gray and behavioural science to (...) present a defence of fear. We show how the pandemic has highlighted deficits in the work of four thinkers highly critical of fear: Martha Nussbaum, Zygmunt Bauman, Hannah Arendt and Sara Ahmed. We argue that, if such approaches are to be of value in anything other than optimal conditions, then they have to acknowledge the fundamental role of fear in helping human beings to pursue fundamental interests. Although ethics consultation services often engage in some of the most complex and delicate clinical situations, little is known about the qual­ity of these services or their effect on patient care and patient and provider satisfaction. There is still significant work to be done in the areas of training, credentialing, and standardization. This article articulates the essential “build­ing blocks” of the Proactive Ethics Integration model developed at Ascension as well as the lessons we have learned along the way.

Published among the posthumous Opuscula of Isaac Barrow in 1687, the Epitome fidei et religionis Turcicae offers an exposition of the main tenets and practices of Islam that is unusually accurate for its time. The Epitome has been noted in passing by Barrow’s biographers and by scholars of seventeenth-century Oriental studies; but it is here firmly identified as the work of the Polish-born Ottoman dragoman and musician Ali Ufki, known in Latin as Albertus Bobovius (Wojciech Bobowski). As the Epitome has (...) never before been studied in detail, this article considers its nature, its importance and its circulation in the European Republic of Letters. The first part treats the evidence of Barrow’s amateur interest in Oriental studies, including his acquisition of the Epitome, and establishes Ufki’s authorship of the treatise. The second part examines the Epitome itself and traces its dissemination and influence through such figures as John Worthington, Samuel Hartlib, William Sancroft, Claes Rålamb and Johan Ulrich von Wallich. Psychiatric disorders can pose problems in the assessment of decision-making capacity (DMC). This is so particularly where psychopathology is seen as the extreme end of a dimension that includes normality. Depression is an example of such a psychiatric disorder. Four abilities (understanding, appreciating, reasoning and ability to express a choice) are commonly assessed when determining DMC in psychiatry and uncertainty exists about the extent to which depression impacts capacity to make treatment or research participation decisions.

Day-to-day life is inundated with attempts to control emotions and a wealth of research has examined what strategies people use and how effective these strategies are. However, until more recently, research has often neglected more basic questions such as whether and how people choose to regulate their emotions (i.e. emotion regulation choice). In an effort to identify what we know and what we need to know, we systematically reviewed studies that examined potential determinants of whether and how people choose to (...) regulate their emotions. Eighteen determinants were identified across 219 studies and were categorised as being affective, cognitive, motivational, individual or social-cultural in nature. Where there were sufficient primary studies, meta-analysis was used to quantify the size of the associations between potential determinants and measures of whether and how people choose to regulate their emotions. Based on the findings, we propose that people’s decisions about whether and how to regulate their emotions are determined by factors relating to the individual doing the regulating, the emotion that is being regulated, and both the immediate situation and the broader social context in which the regulation is taking place. Existing ethical guidelines that aim to guide the development of mental health apps tend to overemphasize the role of Western conceptual frameworks. While such frameworks have proved to be a useful first step in introducing ethics to a previously unregulated industry, the rapid global uptake of mental health apps requires thinking more deeply about the diverse populations these apps seek to serve. One way to do this is to introduce more intercultural ethical perspectives into app design and the guidelines that (...) aim to encourage best practices. In addition to this, existing ethical guidelines can also benefit from the ethical scholarship from the feminist and disability traditions, both of which highlight specific ethical considerations for vulnerable users. Rethinking the ethical responsibilities of mental health app designers through the prisms of feminism, disability studies, and intercultural philosophy leads us to a more global and inclusive set of ethical considerations in app design. This white paper explores what existing guidelines for the regulation of mental health apps are missing. It also explores how these guidelines could be improved for users who inhabit an increasingly diverse and globalized world. (shrink)

Our two-fold goal at this early stage of the paper is to explore some of the different accounts of humility that have traditionally been developed and highlight some of the key debates in the (...) current literature. Next, we present the findings from several studies we recently conducted in an effort to explore people’s intuitions and beliefs about humility as well as their experiences with being humble. Finally, we discuss the relevance of our findings to the ongoing philosophical debates about humility—suggesting that while some varieties of humility are problematic, other varieties of humility are certainly worth wanting. However, a categorical understanding of these associations limits insight into identifying dimensional mechanisms of suicide risk. This study investigated anxious and depressive features through a lens of suicide risk, independent of diagnosis. Latent class analysis of 97 depression, anxiety, and suicidality-related items among 616 psychiatric outpatients indicated a 3-class solution, specifically: a higher suicide-risk class uniquely differentiated from both other classes by high reported levels of depression and anxious arousal; (...) a lower suicide-risk class that reported levels of anxiety sensitivity and generalised worry comparable to Class 1, but lower levels of depression and anxious arousal; and a low to non-suicidal class that reported relatively low levels across all depression and anxiety measures. This article contributes to the genealogy of the concept of academic freedom with a focus on the English universities in the middle of the seventeenth century. It argues that libertas scholastica (the corporate freedom of the universities) and libertas philosophandi (liberty of philosophizing, within and without the universities) were distinctive guiding concepts, sometimes in opposition but occasionally complementary, in debates over the universities in this period. If these two notions together constitute the antecedents of the modern concept of academic freedom, (...) their conjunction must be recognized as a much more contingent and irregular phenomenon than has been previously understood. The co-occurrence of stress-induced posttraumatic stress disorder (PTSD) and obesity is common, particularly among military personnel but the link between these conditions is unclear. Individuals with comorbid PTSD and obesity manifest other physical and psychological problems, which significantly diminish their quality of life. Current understanding of the pathways connecting stress to PTSD and obesity is focused largely on behavioral mediators alone with little consideration of the biological regulatory mechanisms that underlie their co-occurrence. In this work, we leverage prior knowledge to (...) systematically highlight such bio-behavioral mechanisms and inform on the design of confirmatory pilot studies. We use natural language processing (NLP) to extract documented regulatory interactions involved in the metabolic response to stress and its impact on obesity and PTSD from over 8 million peer-reviewed papers. The resulting network describes the propagation of stress to PTSD and obesity through 34 metabolic mediators using 302 documented regulatory interactions supported by over 10,000 citations. Stress jointly affected both conditions through 21 distinct pathways involving only two intermediate metabolic mediators out of a total of 76 available paths through this network. Moreover, oxytocin (OXT), Neuropeptide-Y (NPY), and cortisol supported an almost direct propagation of stress to PTSD and obesity with different net effects. Although stress upregulated both NPY and cortisol, the downstream effects of both markers are reported to relieve PTSD severity but exacerbate obesity. The stress-mediated release of oxytocin, however, was found to concurrently downregulate the severity of both conditions. These findings highlight how a network-informed approach that leverages prior knowledge might be used effectively in identifying key mediators like OXT though experimental verification of signal transmission dynamics through each path will be needed to determine the actual likelihood and extent of each marker’s participation. In Australia, paramedics are obliged to practice ethically. Graduates of baccalaureate degrees in paramedicine should therefore possess a common grounding in ethics to meet the professional capabilities expected of registered paramedics. However, there is a lack of clarity regarding ethics education for paramedicine students, including what is taught, how it is taught, and how it is assessed. This paper explores ethics education for paramedicine students in Australia, how it aligns with current professional expectations, and how it may be enhanced. Point-in-time (...) data regarding ethics education was collected from websites of fifteen Australian universities offering undergraduate baccalaureate degrees in paramedicine. Data collection was supported by consultation with academics from several institutions. Content analysis was utilised to categorise and analyse data to explore similarities and differences in curricula. Similarities included approaches to learning and teaching and the use of case-based learning, with variability found across teaching staff profiles and content areas. Findings suggest it is time for collaboration to develop a model ethics curriculum for paramedicine students in Australia. (shrink)

The study of intellectual humility is still in its early stages and issues of definition and measurement are only now being explored. To inform and guide the process of defining and measuring this important intellectual virtue, we conducted a series of studies into the implicit theory – or ‘folk’ understanding – of an intellectually humble person, a wise person, and an intellectually arrogant person. In Study 1, 350 adults used a free-listing procedure to generate a list of descriptors, one for (...) each person-concept. In Study 2, 335 adults rated the previously generated descriptors by how characteristic each was of the target person-concept. In Study 3, 344 adults sorted the descriptors by similarity for each person-concept. By comparing and contrasting the three person-concepts, a complex portrait of an intellectually humble person emerges with particular epistemic, self-oriented, and other-oriented dimensions. Recent studies have revealed an astonishing diversity of sex chromosomes in many vertebrate lineages, prompting questions about the mechanisms of sex chromosome turnover. While there is considerable population genetic theory about the evolutionary forces promoting sex chromosome replacement, this theory has not yet been integrated with our understanding of the molecular and developmental genetics of sex determination. Here, we review recent data to examine four questions about how the structure of gene networks influences the evolution of sex determination. Microbial genome sequencing has entered a new phase, where DNA sequence information is gathered from entire microbial communities (metagenomics or environmental genomics) rather than from individual microorganisms. By providing access to the genetic material of vast numbers of organisms, most of which are organisms that have never been isolated or cultivated, a new level of insight is being gained into the diversity and extent of the microbial processes that are presently occuring in environmental communities. By extending metagenomic‐based approaches to the (...) study of very complex and methodologically recalcitrant soil environments, a recent study has found that ammonia‐oxidizing archaea are more abundant in many soils than bacteria.1 These findings not only highlight the undoubtedly critical yet unknown roles that archaea play in global nutrient cycles but illustrate the importance of genomic studies for informing us about the functional capacity of life on Earth. We prove that the generalized cancellation axiom for incomplete comparative probability relations introduced by Rios Insua and Alon and Lehrer is stronger than the standard cancellation axiom for complete comparative probability relations introduced by Scott, relative to their other axioms for comparative probability in both the finite and infinite cases. This result has been suggested but not proved in the previous literature.

Matthew Arnold was born at Laleham-on-Thames on 24 December 1822 as the eldest son of Dr Thomas Arnold and his wife Mary. He was educated at Winchester College, his father's old school; Rugby, where his father was headmaster; and Oxford. In 1851 he was appointed Inspector of Schools, pursuing this taxing career to support his wife and family until his retirement in 1886. He published his first volume of verse, The Strayed Reveller, and other Poems, in 1849 followed (...) by Empedocles on Etna, and other Poems (1852) and five further collections which appeared, with a diminishing number of new poems in each, between 1853 and 1867, after which his creative gift appeared to dwindle still further and he published little poetry. His career as a writer of prose began to take over after his election to the Professorship of Poetry at Oxford in 1857. Stimulated by preparing his lectures, many of the earliest published in 1865 as Essays in Criticism (First Series), he turned increasingly to the vigorous and widely ranging polemical commentaries on culture, religion, and society which were to make him known at home and abroad as the foremost critic of his day. He died suddenly of heart failure on 15 April 1888 while awaiting at Liverpool the arrival of his married daughter from America. (shrink)

The essays in this book, by a variety of leading Augustine scholars, examine not only Augustine's multifaceted philosophy and its relation to his epoch-making theology, but also his practice as a philosopher, as well as his relation to other philosophers both before and after him. Thus the collection shows that Augustine's philosophy remains an influence and a provocation in a wide variety of settings today.

Due to natural selection pressure, certain aspects of memory may have been selected to give humans a survival advantage. Research has demonstrated that processing information for survival relevance leads to better item memory (i.e., the content of information) compared to control conditions. The current study investigates the effects of survival processing on context memory (i.e., memory for peripheral episodic details) and item memory to better understand when the survival processing memory advantage emerges. In this study, participants viewed objects in either (...) a plausible color, for example a red apple, or in an implausible color, such as a green pie. We chose this color plausibility manipulation because color is a detail that conveys information about the fitness of an item. After studying items in either a survival or moving (control) condition, participants made item memory judgments (did you see this item before?), and two context memory judgments: color context (in which color did you see this item?), and source context (in which condition did you see this item?). Results indicated better item memory for materials processed in the survival relative to moving condition. Critically, for color context there was a condition by plausibility interaction, where memory was best for plausibly colored items in the survival processing condition. There was no difference, however, in source context memory between the survival and moving conditions. These results suggest the survival processing memory advantage extends to some contextual details, but only those details that tap into the survival utility of items, such as color. (shrink)

The problem of inferring probability comparisons between events from an initial set of comparisons arises in several contexts, ranging from decision theory to artificial intelligence to formal semantics. In this paper, we treat the problem as follows: beginning with a binary relation ≥ on events that does not preclude a probabilistic interpretation, in the sense that ≥ has extensions that are probabilistically representable, we characterize the extension ≥+ of ≥ that is exactly the intersection of all probabilistically representable extensions of (...) ≥. This extension ≥+ gives us all the additional comparisons that we are entitled to infer from ≥, based on the assumption that there is some probability measure of which ≥ gives us partial qualitative information. We pay special attention to the problem of extending an order on states to an order on events. In addition to the probabilistic interpretation, this problem has a more general interpretation involving measurement of any additive quantity: e.g., given comparisons between the weights of individual objects, what comparisons between the weights of groups of objects can we infer? Paramedics encounter ethical dilemmas at work, and while previous research has improved ethics education and practice, more can be learned from the lived experience of paramedics facing ethical challenges. This paper explores the lived experience of two paramedics, one with five years’ experience, the other twenty, presented with comparable cases. The participants, who were interviewed in a broader qualitative study exploring practical wisdom, were asked to select a case involving ethical decision-making from their own practice experience. Semi-structured interviews employed the (...) five-step Critical Decision Method as a framework to explore various aspects of the decision-making process. The two cases were identified as exemplars of prominent themes suitable for discussion. The findings highlighted several factors that influence paramedic ethical decision-making, including practical wisdom, communication and teamwork, clinical support, and education. This case comparison identifies a greater need to focus education on the fostering of practical wisdom, and the development of non-technical skills such as communication and teamwork, supported by interprofessional education endeavours. Furthermore, greater access to support for paramedics must be available to further evolve practical wisdom. The combination of improved education and support will likely cater for the full spectrum of ethical challenges faced by paramedics in their work. (shrink)

Special focus is given to the series of Latin, Senecan sententiae showcased in scenes 11 and 13, as More reacts to his fall from high office and worldly fortunes. By means of this analysis, the article offers further insight into the remarkable character of the play's Thomas More, namely his (...) habit of balancing tragic and Senecan attitudes with more comedic ones in order to play the well-prepared role of a comic actor, despite a tragic stage. The contributors’ efforts to bring to light the insights of Aquinas are organized into three sections: theological method, sacramental theology, and moral theology.Readers of this journal will be most interested in the latter section. Highlights include Craig Steven Titus’s response to Jean Porter’s criticism of Aquinas’s doctrine of the unity of the virtues, in which Titus claims that Aquinas has a more developed awareness of moral struggle and development, even among the virtuous, than Porter allows, and that therefore Aquinas is not averse to the idea of a “flawed saint” (331). Steven A. Long distinguishes between acts that are necessary for the completion of a given end and those that are not, arguing that for the former the end defines the species of the act whereas the latter possess their own end and species independent of further intentions. Long’s purpose is to refute Martin Rhonheimer’s contention that condoms can be used by married couples to prevent the spread of HIV/AIDS by showing that the use of condoms is not necessary for this purpose (since abstinence is also available), and therefore that such use must be considered an act of contraception regardless of any other good intentions involved. Joseph W. Koterski provides an excellent overview of the classical roots of the natural law theory and its modern exponents that could be useful in undergraduate and graduate courses, although the essay seems mismatched for the volume since the likely reader will probably find little new in it.An evaluation of the overall project of the volume requires a reading of the essays in the first section on theological method. Thomas Joseph White points to the Dominican theologians Marie-Dominique Chenu and Reginald Garrigou-Lagrange as two extremes in the debate over historicity in theology: “in one we find history without sufficient recourse to dogma, while in the other we have dogma without sufficient recourse to history” (96). White insists that [End Page 218] theologians must find the proper balance between the two. It is not entirely apparent that those loosely identified with ressourcement Thomism have found this balance, if “recourse to history” includes sensitivity to the questions and concerns of one’s own age. In many of the essays in this volume it is unclear whose questions the authors’ exegeses of Aquinas are meant to answer, although other essays are quite engaging. Of course historical exegesis is necessary, but those involved in the mid-twentieth-century patristic ressourcement understood that the recovery of the sources was necessary precisely as an answer to the burning questions of the contemporary world. This is not accommodation, since the Christian tradition proposes something new to the world, but it proposes it with the confidence that human questioning is rooted in the innate desire for the triune God. The advocates of the earlier ressourcement saw the Thomism of their day as an obstacle to this dialogue between the Christian tradition and the contemporary world. The project of ressourcement Thomism will succeed to the extent that it shows how the teachings of Thomas Aquinas can contribute to this dialogue. To many, Thomas’ critique represents no advance beyond Gaunilo in understanding (...) Anselm, but merely reproduces Gaunilo’s objection against Proslogion III in Pro Insipiente VII, and his critique has even evoked from one of the most influential modern proponents of Anselm’s argument the remark "This is not very perceptive, is it?" Such an attitude has, as one might expect, hardly been modified by the satisfaction with which Thomists often regard Thomas’ rebuttal while ignoring both the logic of the argument and the details of Thomas’ reply to it.
